About Social Work Services
Social work services are available to both clients and their families. The hospice social worker can assist a family in a number of ways, including:
- Grief support and counseling, including assisting family members in coping with loss issues and care giving demands
- Assistance in finding community support resources to allow for them to care for their loved one
- Education regarding the dying process
Bereavement Outreach & Support
Some of the bereavement outreach and support services provided by Jefferson County include:
- Providing up to 18 months of follow-up services for hospice families, as well as ongoing community bereavement services
- Making available individual and family counseling
- Sharing bereavement education and information
- Scheduling monthly support group meetings for the bereaved
- Connecting client and family with community resources
