About the Interdisciplinary Team
Interdisciplinary team (IDT) meetings are held bi-monthly to discuss current hospice clients' needs and their progress towards goals.
Team members include the hospice medical director, RN case managers, social work services, chaplains, volunteers, home care aides, the client, and the client’s caregiver and family members.
The hospice IDT assesses the needs of hospice clients and their families by providing palliative and supportive care through a collaborative effort. These needs may include physical, psychological, spiritual, social, or economic stress that may be experienced by the client / client’s family members during the client’s illness, dying process, or during the bereavement period.
